Notes:

* Payments due dates and amounts are non-negotiable. In order to secure a uniform all players must pay fees on time and in full. Payments that are late or not received by the team will result in $5 per week late fees and loss of playing time.

* The final payment is due on 5/18/10.

** Uniform orders will be placed on 4/21/10. If your first (2) payments are not in by the 18th of April, you will not be able to secure a jersey number and this will result in a delay in your uniform order. If you do not have a uniform you cannot play.
